Stop Use Field Service Action - 3M™ DBI-SALA® Free Standing Counterweight Anchor Using an Evolution Leap Post

3M Fall Protection (formerly Capital Safety) has received field reports of broken end-posts (also known as Lead Energy Absorbing Posts, or “LEAP posts”) used in roof-mounted EvolutionTM Horizontal Lifeline Systems. 3M has launched a global Stop Use - Field Service Action to address this situation. The same Evolution end-posts are also used in Free-Standing Counterweight Anchor systems. There have been no reports of fall-related injuries associated with this condition.

To address the Free Standing Counterweight Anchor situation, 3M is launching a global Stop Use – Field Service Action to retrieve and replace all end-posts used in these Anchors sold between 2005 and 2015 (at which time the Evolution end-post was discontinued and replaced with a Roof Safe end-post).

The Free Standing Counterweight Anchor System is designed as a portable means to anchor a personal fall arrest system for a person working on flat roofs or structures. The P/N’s affected include 7226000, 7240313, 7255000, 7255002 & 7255003.

End-users: Please inspect your counterweight anchor. If it has an Evolution Leap Post, please take this anchor out of service immediately and use an alternative fall protection solution. Then contact our Customer Service department at 800-328-6146 (prompt #2012) or email 3MUSFPServiceAction@mmm.com and a replacement post and base plate # 7255006 will be shipped to you free of charge. Install the replacement post and baseplate into the counterweight as described in the user manual and follow instructions for returning the affected post using the pre-paid mailing box.

If you have any affected systems in stock, you should return the systems to 3M Fall Protection for replacements. Contact our Customer Service department at 800-328-6146 (ext. 2012) or email 3MUSFPServiceAction@mmm.com to obtain a “QA” number to return the unit to 3M Fall Protection.
