3M software was used by a Minnesota consortium in a collaborative effort to reduce hospital readmissions in the state of Minnesota. The Reducing Avoidable Readmissions Effectively (RARE) campaign was one of the largest coordinated improvement initiatives within the state health care community, involving 82 hospitals and 100 community partners, with a focus to keep patients from being readmitted to the hospital when it could have been avoided.
Not all readmissions are preventable, but avoidable readmissions represent two to five percent of the inpatient hospital costs in the United States. The RARE campaign utilized 3M™ Potentially Preventable Readmission (PPR) software, which identifies avoidable readmissions that are the result of poor hospital care, discharge disposition and/or follow-up care, to know where to focus efforts to reduce the readmissions that could be prevented.
As a result of the program, 6,211 avoidable readmissions were prevented over a 30-month period, meaning that patients spent 24,844 more nights sleeping comfortably in their own beds, instead of the hospital. The program also reduced inpatient costs by $55 million.
