Reimagining care delivery

Looking beyond clinical data

  • As we head into 2021 after an unprecedented year, in an industry already plagued with disparities in access and inefficiencies in delivery, we are anticipating how care delivery needs to be reimagined to create a new framework to reward better care.

Start off the new year by joining 3M experts on Jan. 19, as they discuss how to target both individuals and communities facing health care disparities, and why it is a necessary backdrop to real change.

    Attendees will learn how to

    • Continue building efforts that improve access to affordable, quality care and achieve health equity
    • Use analytics that combine social and clinical risk
    • Identify and target more clinically vulnerable populations through social risk and create a model to achieve improved outcomes
