3M CDI Innovation Webinar

Leveraging technology to capture HCCs across the continuum

May 27, 2020

  • As health care moves toward risk-adjusted models for care delivery, you can’t ignore Hierarchical Condition Categories (HCCs), especially in outpatient settings. And you need to adapt to ongoing changes, such as new regulation from CMS in April allowing submission of diagnoses via telehealth.

In this webinar, 3M experts Colleen Deighan, RHIA, CCS, CCDS-O, 3M senior outpatient CDI consultant, and Dan Engel, 3M HCC management leader, explore strategies to use artificial intelligence (AI) technology and workflow tools to effectively manage HCCs. Attendees will learn how to:

    • Increase their understanding of industry best practice for managing HCCs across the continuum of care.
    • Provide real-time guidance to physicians while documenting in the EHR to close HCC gaps before submitting claims.
    • Help CDI teams identify patients for annual follow-up visits (in person or telehealth) to avoid missing high-risk conditions.
    • Enhance workflow for CDI teams to review more patient records pre-visit and share details with clinicians.

Our presenters explore HCC innovations that can ease the burden on physicians at a critical time.
