    3M DTA offers data extracts that cover population health, facility quality and payment, and episodic care. Cost analysis (inpatient and outpatient), pricing (Medicare) and attribution (primary care physician, specialist and facility) propriety value-added enhancements are included in selected data extracts.

    3M DTA converts client’s claims, eligibility and provider data into a set of integrated data extracts enriched with 3M grouper methodologies and proprietary data output that help address the population health and payment challenges health care payers and providers are facing.
  • The typical 3M DTA client is a health plan or accountable care organization (ACO) that utilizes population health, facility quality and payment, and episodic care data to improve health care quality and cost.
  • With 3M DTA, clients can save time and resources processing, curating and integrating 3M grouper output by receiving structured extracts integrated data enriched with 3M grouper output and other proprietary data output specifically designed to understand and address health care quality and cost.

The client submits data according to the 3M data input specification document. Full and final claims data is submitted as one data feed in accordance with HIPAA, HITECH, as well as state and federal privacy laws.

  • 3M All Patient Refined DRGs
    3M APR DRGs classifies hospital inpatients according to their reason for admission, severity of illness and risk of mortality.
  • 3M Enhanced Ambulatory Patient Grouping
    3M EAPGs were designed to reflect the resources used in an ambulatory visit. These include nursing and technician time, drugs, supplies, etc. Patients in each 3M EAPG share similar clinical characteristics, resource use and costs. EAPGs are not applicable to services of physicians and other professionals typically paid separately.
  • 3M Clinical Risk Grouping
    3M CRGs describes the health status and burden of illness of individuals in an identified population. The optional 3M Functional Status Groups (FSG) methodology supplements 3M CRGs when individuals have limitations in performing the activities of daily living.
  • 3M Potentially Preventable Complications
    3M PPCs define harmful events (like an accidental laceration during a procedure) or negative outcomes (such as hospital-acquired pneumonia) that may result from the process of care rather than the natural progression of a disease.
  • 3M Potentially Preventable Readmissions
    3M PPRs are a readmission within a specified time interval that are determined to be clinically related to a previous admission and potentially preventable.
  • 3M Patient-focused Episodes
    3M PFEs are a categorical, clinical model that defines episodes of care to reflect a patient’s total burden of illness, not merely the presence of a single disease. 3M PFEs simultaneously quantify the patient’s acute and post-acute resource needs, taking into account both the immediate need for care and baseline health status. The methodology was designed for purposes of payment, utilization analysis and clinical insight.
  • 3M Potentially Preventable Admissions
    3M PPA identifies hospital admissions that could potentially have been dealt with in the outpatient setting. These hospital admissions may result from hospital and/or ambulatory care inefficiency, lack of adequate access to outpatient care, or inadequate coordination of ambulatory care services.
  • 3M Potentially Preventable Services
    3M PPS addresses high-cost ancillary services that may not provide useful information for diagnosis or treatment, and therefore have no effect on clinical management. Such services include diagnostic tests, laboratory tests, therapy services, radiology services and pharmaceuticals that may be redundant or are not reasonably necessary for providing care or treatment.
  • 3M™ Ambulatory Potentially Preventable Complications (AM-PPCs)
    3M AM-PPCs are complications of care (e.g., pneumonia, sepsis) following an ambulatory procedure that may result from processes of care and treatment rather than from natural progression of an underlying illness and are therefore potentially preventable.
