When requested and as required by applicable law, 3M will provide an exemption for employees’ religious beliefs, practices, and observances, provided the requested exemption is reasonable and does not create an undue hardship for the Company to the extent permitted by applicable law.

    If you are requesting an exemption related to a vaccination (including a COVID-19 vaccination) based on your religious belief, practice, or observance, please, complete this form and submit it.
