This presentation will provide attendees with an in-depth understanding of risk adjustment and risk methodologies such as Hierarchical Condition Codes (HCCs), including how and why they are employed today in managing the health of populations, with special emphasis on the use of HCCs to drive value-based care. The session will help attendees answer the question: As the stewards of health information, how can HIM professionals help ensure that the health of a population is accurately reflected and help and reduce overall cost across the continuum of care?

Donna Smith, RHIA, is a Senior Consultant and Project Manager with the Consulting Services business of 3M Health Information Systems. As a member of the 3M development team, she participates in the design, development, project management, and delivery of all 3M consulting services for ICD-10, clinical documentation improvement, risk adjustment, and services to support new payment models and population health management. Previously, she was 3M project leader for documentation assessments, CDI program implementations and coding validations. Donna is an AHIMA approved ICD-10-CM/PCS Trainer, and is a nationally recognized expert on HIM emerging issues. She has given numerous presentations and practical workshops at national and regional industry conferences. Donna is a graduate of Indiana University with a degree in health information management administration, and has over 35 years of experience working in the health information management field.
